At Tennis Camp, your child will work on all of the varieties of shots and strategies. Camp is open to
children entering grades 3-8 and of all abilities. Each participant should please bring a water bottle,
towel, proper shoes and sunscreen. We will meet at the high school tennis courts each morning.

activities and events, and that my authorization does not conflict with any medical advice or concerns expressed
by my child’s physician. None of the coaches are qualified physicians. Waiver of Liability: Participation in the
camp is at your own risk and neither the coaches, the hosting school district nor any volunteer shall be liable for
any injury or damages resulting from participation in the camp. By signing this form, you on behalf of yourself and
your child or other persons for whom you are legal guardian, confirm (1) That you understand the statements
contained on this form and (2) That you release the coaches, the hosting school district and volunteers from any
claim, liability or damages occurring during this game. I give the camp permission to take pictures of my child and
