Mercy Health Orthopaedics and Sports Medicine is offering state required Pre-Participation Sports Physicals from 8 a.m. to 10 a.m. on Saturday, June 4 at Mercy Anderson HealthPlex for anyone in grades 7-12 who plans to participate in middle school or high school athletics during the 2016-17 school year. Cost is $20 and dress is shorts and T-shirt for the examination.
Parents or guardians must fill out and sign a Physical Examination Consent Form available at http://www.nrschools.org/Downloads/Physical_Exam_Consent_Form.pdf and complete Pages 1, 5 and 6 of the Ohio High School Athletic Association Pre‐participation Physical Evaluation Form which is available for download at http://www.nrschools.org/Downloads/PPE_2016-17.pdf or http://ohsaa.org/medicine/PPE_2016-17.pdf. Pages 3 & 4 of the PPE form will be filled out by the Mercy Health medical staff.
