New Richmond High School through Healthsource Ohio is offering state required physicals for high school and middle school athletes for the 2014-15 school year from 4 to 6 p.m. Wednesday, June 4 at the high school. Cost is only $10 per physical which will be conducted outside the down stairs locker rooms.
Parents or guardians must fill out and SIGN the Ohio High School Athletic Association Preparticipation Physical Evaluation which can be downloaded at http://www.nrschools.org/Downloads/PPE_2014-15.pdf) and have their student athlete bring it along with payment of $10.
