OHYES! Parent Letter and Opt-Out Form

Please click on MORE to learn about the upcoming Ohio Healthy Youth Environments Survey that will be administered to students in grades 7-12.

The New Richmond Exempted Village School District is working hard to better understand youth health risks and protective behaviors in the community. 


As part of this effort, the district will be asking students in grades 7-12 to participate in an anonymous survey called the Ohio Healthy Youth Environments Survey (OHYES!). Only students age 12 or above are eligible to participate.


The OHYES! is a tool designed by the Ohio Departments of Education, Health, and Mental Health and Addiction Services and numerous other programs advocating for youth. Our district has decided to use this survey to understand more about our students. A letter will go home soon to the families of students in grades 7 through 12 explaining the survey and offering parents the opportunity to opt their student out of the survey if they wish. 


For more information, please click on the Parent Letter/Opt-Out form that was emailed on April 23 to all families with students in grades 7-12 who have current contact information on file with their student's school.
